Wellness Director (DNS)
Responsible for the coordination and oversight of health and supportive services provided to residents at an Assisted Living Community. Responsible for the day-to-day clinical operations of the community to ensure resident’s healthcare needs are met. Provides training, supervision, and monitoring of associates in the administration of direct care within the community. Performs ongoing assessment/observation of residents' physical and psycho-social needs and coordinates with other departments to assure quality, proactive care. Interacts directly with associates, residents, resident family members, vendors and professional contacts within the community.

Enrichment Director
The Director will develop, implement and conduct a meaningful life enrichment program based on resident’s interests, needs and abilities to enhance resident wellness and quality of life. Develop a monthly calendar that displays this program to the residents, staff, families, visitors and volunteers. Interact with the greater community recruiting volunteers and facilitating community involvement.

Dining Services Aide
*** $1,000 Sign-on Bonus ***
The Server delivers food from the area of preparation to the residents at their tables and/or apartment. The position will be responsible for ascertaining that the residents are satisfied with the menu items, providing beverages and re-fills as needed as well as providing condiments and other items the resident may request. The position is responsible for bussing tables, changing table linens and resetting tables following meal service as well as pouring beverages and setting up tables prior to each meal service.

Caregiver
***$1,500 Sign-on Bonus!**: (full-time only)
The caregiver is responsible for personal services, medication assistance, delegated tasks, housekeeping, laundry, social-recreational activities, meal services and others tasks as needed for resident well-being.

Cook
*** $1,500 Sign-on Bonus ***: (full-time only)
The cook position prepares and serves meals. Oversees food service operation and ensures that adequate food and supplies are on hand.

RECOVERY COACH (Entry Level Mental Health Worker) ROLE:

Under the supervision of the Director of Nursing and the Program Director, the Recovery Coach (Entry Level Mental Health Worker) is responsible for assisting in the provision of the therapeutic milieu by monitoring and communicating with clients, providing rehabilitation groups under supervision, and assisting in crisis intervention. Other duties for the Recovery Coach (Entry Level Mental Health Worker) may be assigned as needed by the Director of Nursing, the Program Director, or a Designee.

The Recovery Coach (Entry Level Mental Health Worker) is a critical part of the team ensure proper care over our clients. Many Recovery Coach (Entry Level Mental Health Worker) go on to be promoted to Service Coordinator roles.

POSITION QUALIFICATIONS:

  • Minimum of a high school diploma/GED.
  • A valid California driver’s license preferred.
  • Minimum of two years clinical experience as applicable in a psychiatric setting.
  • In the absence of two years experience, training may be substituted. This training will be based on, but not limited to, the in-service plan for 52 hours.
  • Driving duties may be assigned, including the transportation of clients. Candidates must have a valid CA Drivers License and successfully pass a DMV driving history check.

The Licensed Vocational Nurse (LVN) / Licensed Psychiatric Technician (LPT), Registered Nurse (RN) provides direct nursing care to the clients according to physicians’ orders and applicable client care standards, within their scope of practice. The Nurse will provide direction to non-licensed personnel in providing direct client care and operation of the unit.

POSITION QUALIFICATIONS:

Current licensure in California as a RN, LPT or LVN
Knowledge of general nursing theory and practice.
Ability to supervise.
Ability to work cooperatively with other staff members.
Previous experience in mental health nursing preferred.
Current CPR and first aid certifications.
Knowledge of DBT, WRAP and Psychosocial Rehabilitation Principals preferred

POSITION RESPONSIBILITIES:

Directs the day-to-day functions of the nursing activities in accordance with all applicable regulatory standards.
Supervises assigned staff.
Assesses, plans, implements, and evaluates nursing care for clients. Works with the service coordinator in developing service plans for the client.
Follows protocols regarding medication administration and treatments per physicians’ orders.
Documents all pertinent information regarding the client’s condition, medication, treatment, and communication with legal and health care representatives per policy.
Supervises the use of restraints used for physical and behavioral reasons, ensuring that the client’s rights and safety are upheld.
Communicates with appropriate personnel and disciplines on a daily basis regarding admissions, discharges, changes in condition, physicians’ orders, and other appropriate information.
Notifies physicians of all incidents, changes in condition, and need for evaluation of all medications or other treatment in a timely manner.
Assures conservator and/or family members are notified of any incidents or changes in condition as appropriate.
Acts as a liaison with the physician, client, and all members of the interdisciplinary team.
Completes incident reports, medication error reports, and drug reaction reports in a timely manner per protocol.
Makes independent nursing decisions as necessary based on appropriate nursing theory and practice.
Observes proper safety practices and recommends or initiates plan of correction as necessary.
Upholds all infection control standards.
Participates in the implementation of quality assurance programs for nursing services.
Maintains confidentiality of all client care information.
Keeps Director of Nursing Services informed of all potential problems on the unit.
